1.) To expedite your check in process the morning of the your pet's procedure, please fill out and bring BOTH of the following forms with you:

Admit/Registration Form

Dental Cleaning Consent Form

2.) Remove all food and water sources from your pet by 10pm the night before their cleaning as a safety precaution for anesthesia.

3.) Give prescribed medications as usual that morning.

4.) Arrive at the hospital at 8am to check in so we can begin patient prep.
