We are excited you have decided on Adair County Animal Hospital
​and Laser Surgery Center for your pet's surgery, boarding, daycare,
and grooming accommodations!
In order to expedite your check-in:

  • ​Please print and fill out the information listed on the Adair County Animal Hospital Registration/Admission Form (attached below)
  • Bring Registration/Admission Form with you at check-in.
  • Bring documentation of any required vaccines or test results (listed below) performed by any other licensed veterinarian.​
Daycare/ Boarding: ​

Please let us know on the Registration/Admit Form if you would like your pet to have any of the following services:
​
  • Extra playtime during the day, either alone or with human interaction (additional charges apply for each 15 minute increment.) 
  • List any medications that need to be given and the frequency they are given each day (additional charges apply.)
  • ​Please label any items you are leaving with your pet.

Your pet is required to be current with the following items
​for boarding, daycare, and grooming reservations:
CANINES:
Required Annually:
Required every 6 months:
  • Leptospirosis  (L4)
  • Influenza
  • Bordetella  (Kennel Cough)
  • Fecal  (intestinal parasite check)
FELINES:
Required Annually:
  • ​Rabies 1-year *
  • 4 in 1 or FVRCP​ - main feline vaccine
Required every 6 months:
To keep our facility flea-free, all pets that are dropped off for any services are given a Capstar, 24-hour flea pill.
We want all of the pets in our care to remain as healthy as possible before, during, and after their stay with us! 

*This is the only vaccination required for surgery/ hospitalized patients to maintain the health and safety of our doctors and staff. It is also required annually by Kentucky State law.
